The following personal data is collected and processed to determine your current health status, and whether you have had any recent COVID-related exposure.  Where required by the government, and pursuant to Republic Act No. 11332 (“Mandatory Reporting of Notifiable Diseases and Health Events of Public Concern Act”), DOH and NPC Joint Memorandum Circular No. 2020-02 (“Privacy Guidelines on the Processing and Disclosure of COVID-19 Related Data for Disease Surveillance and Response”), and the DTI and DOLE Interim Guidelines on Workplace Prevention and Control of COVID-19, we may submit your personal data to the Public Health Authority for contact tracing and monitoring and mandatory reporting  purposes. We shall keep a copy of these records for a period of sixty (60) days from the date of your visit. After the retention period, we shall securely destroy all forms submitted.
